If you are a patient at our practice and either:
• Do not have health insurance
• Prefer not to use your health insurance
• Have health insurance that does not pay for certain services
A membership plan is a perfect fit for you. Plans include preventive care and discounts on other procedures.
This program still requires the Doctor to evaluate you, and should they accept you a a patient, then you would have an established patient relationship with our practice.

Yes! You can easily sign up additional family members for their own membership plan!
Your membership plan is between our practice and you; it is not transferrable to another Doctor.
